• To self-schedule for in-person appointments for tomorrow, or later in the week, please use the following links for Well Visits (for example, questions for the nurse, chronic health conditions, sexual health, or contraceptive planning) or Sick Visits (for example, cough, sore throat, fever, nausea & vomiting). A nurse will contact you on the day prior to your scheduled appointment to confirm your appointment.
